La utilización de recursos sanitarios en pacientes infectados por el virus de la inmunodeficiencia humana: creación de una base de datos y obtención de resultados de costes

Objetivos

Elaborar y validar una base de datos que permita recoger información acerca de la evolución clínica y el consumo de recursos sanitarios de pacientes infectados por el Virus de la Inmunodeficiencia Humana (VIH). Obtener datos sobre costes de la utilización de recursos a partir de la información recogida durante el proceso de validación.

Métodos

Se ha diseñado una base de datos, en soporte informático, que consta de varios apartados: datos demográficos, datos clínicos y datos de utilización de recursos asistenciales. Se han seleccionado 109 pacientes de cinco hospitales españoles y se han seguido durante seis meses (enero a junio de 1997). Los costes de los recursos sanitarios consumidos se han obtenido de la contabilidad analítica de un centro hospitalario y de tarifas oficiales.

Resultados

El soporte informático diseñado es un instrumento útil y apropiado para recoger la información de estos pacientes. El coste anual promedio es de 2,5 millones de pesetas, variando, según la fase de la enfermedad, desde 1 millón de pesetas para los asintomáticos, hasta 3,8 millones para los pacientes en fase sida. La comparación de los costes resultantes con otros estudios anteriores muestra un significativo incremento de los costes en los pacientes asintomáticos.

Conclusiones

Los resultados constituyen una primera información sobre la utilización de servicios sanitarios por pacientes infectados por el VIH y confirman la viabilidad de trabajar, en un futuro, con el instrumento diseñado. La implantación de un sistema de seguimiento es un imperativo si se desea disponer de un sistema de información rápido y preciso sobre la evolución de variables clínicas y de sus repercusiones económicas en pacientes de una enfermedad de pronóstico incierto y cambiante.

Objectives

To make and validate a data base that allows to collect information about clinical characteristics and use of health care services by HIV-infected patients. An additional purpose is to obtain some data about costs of health care services utilization.

Methods

We have designed an informatic data base that includes: demographic data, clinical data and health care services utilization during the follow up. One hundred and nine patients were selected from 5 different hospitals in Spain and they were followed for 6 months (from January to June 1997). Analytic accounting of one hospital and official tariffs were used to calculate costs of the health care services.

Results

The informatic data base designed is useful and appropiate to collect the information of these patients. The annual average cost is 2.5 milions pesetas, ranging from 1 milion pesetas for asyntomatic patients to 3.8 milions for AIDS patients. The comparison of our results with previous studies shows an increased cost of asymptomatic patients.

Conclusions

These results give a preliminary information about health care services utilization by HIV-infected patients and confirm the possibility of using the designed instrument in the future. Since HIV-infection presents an uncertain evolution and a variable prognosis, to implant a tracking system is a necessary requirement in order to obtain a fast and accurate information system about the evolution of clinical variables and their economic repercussions.
