Brotes de sarampión: consideraciones a propósito de los brotes investigados en Cataluña (1990–1995)

Objetivos

Determinar los factores asociados con el tamaño de los brotes de sarampión en Cataluña.

Métodos

Se estudiaron los brotes del período 1990–95. Se compararon los brotes de más de seis casos con los de dos a seis casos en función de las variables: mediana de edad de los casos, ámbito de transmisión y año de presentación. Mediante regresión logística múltiple se valoró el efecto independiente de cada variable, obteniéndose las odds ratio ajustada (ORa) y sus intervalos de confianza.

Resultados

Se estudiaron 45 brotes, con un total de 595 casos de sarampión. El 39,5% de los brotes se presentó en el grupo de 6–10 años. Los brotes de más de seis casos tuvieron una probabilidad superior de presentarse en el ámbito escolar (ORa = 4.3, IC 95% 1,0–17,6), pero no se asociaron con la edad (ORa = 2,1, IC 95% 0,5–9,2) ni el año de presentación (ORa = 0,9 IC 95% 0,2–3,6)

Conclusiones

El ámbito de presentación (escuelas) determina el tamaño de los brotes de sarampión y requiere la aplicación de estrategias preventivas diferenciadas.

To determine the factors associated with outbreak size in Catalonia.

Methods

Outbreaks over the period 1990–95 were investigated. A comparation was run between outbreaks of more than 6 cases with respect to outbreaks of 2–6 cases in function of the following variables: median age of cases, transmission setting and year of presentation. Multiple logistic regression was used to asses the independent effect of each variable, with the ajusted odds ratio (ORa) and their 95% confidence interval (CI) being obtained.

Results

In all, 45 outbreaks were studied, with 595 cases of measles 39,5% of outbreaks ocurred in the 6–10 age group. Outbreaks of more than 6 cases had a higher likelihood of appearing in a school setting (ORa = 4.3, CI 95% 1,0–17,6); but there were no association with age (ORa = 2,1, CI 95% 0,5–9,2) or occurring prior to 1994 (ORa = 0,9, CI 95% 0,2–3,6).

Conclusions

School setting determines the outbreak size of measles and demands to apply specific preventive strategies.
